What is a biomarker?

A biomarker is a measurable signal in your tumor or blood. In kidney cancer, the most important biomarker is a gene called VHL — when it’s lost from cancer cells, it triggers a chain reaction that targeted drugs are designed to stop.

Kidney cancer is no longer treated primarily with chemotherapy — it doesn’t work well. Instead, targeted drugs and immunotherapy combinations are now standard. Biomarker testing helps your doctor choose the right combination for you.

What this means for you: Your doctor will use your risk category and biomarker results together to choose the best first-line treatment — often a combination of a targeted drug and immunotherapy.

VHL loss is found in most clear cell kidney cancers. When VHL is lost, the cancer sends a signal to grow new blood vessels to feed itself. Drugs like sunitinib and cabozantinib block that signal directly.

Do I need chemotherapy for kidney cancer?

Standard chemotherapy does not work well for kidney cancer. Treatment is based on targeted drugs and immunotherapy. Your doctor will choose based on your risk category and other factors.

What is the IMDC risk score?

A score based on six simple factors — like your hemoglobin and calcium levels — that classifies your risk as favorable, intermediate, or poor. Your risk category is one of the main things that determines which first-line treatment is best for you.

What if I have non-clear cell kidney cancer?

About 20–25% of kidney cancers are non-clear cell types. These have different biology and may respond differently to treatment. Ask your doctor to confirm your specific tumor type.
